Cancer survivorship begins at the time of diagnosis and lasts through the duration of life. Survivor Healthcare serves any adult 18 years and older who has been diagnosed with cancer, regardless of the stage or type.

What We Treat
Cancer is tough. We are all aware of the stigma and stress that follows a cancer diagnosis and treatment. For many patients, there are a myriad of additional underlying and lingering issues connected to their disease.

How We Do It
A Collaborative Approach to Care
To understand our patient's health-related quality of life (HRQoL), we collect self-reported data (PROs) on their mental, physical, social, and financial well-being. Statistical algorithms analyze this data and generate scores that serve as thresholds for triggering alerts. This data-driven approach allows healthcare providers to identify areas where a patient's HRQoL might be suffering. By analyzing trends and changes in these scores, treatment plans can be modified to better address a patient's specific needs and improve their overall well-being.
